In the aftermath of the 2008 financial crisis, Queen Elizabeth II visited the London School of Economics and asked a simple question: why did nobody see the collapse coming? The eventual response from a group of economists and academics was striking. The failure, they wrote, had been “a failure of the collective imagination . . . to understand the risks to the system as a whole.”
That phrase has stayed with me for years because it captures something much larger than finance. It speaks to a recurring pattern across organizations, institutions, governments, corporations, and even entire societies: We become extraordinarily skilled at managing the ‘parts’ while losing sight of the relationships, patterns, and overall structure of the system.
If you look online for systems-related professional development opportunities today, many are aimed at actuaries, risk professionals, strategists, and resilience practitioners. This makes sense. Systems thinking is, in many ways, the study of interdependence, adaptation, and unintended consequences. It is about learning to perceive risk not merely as isolated events, but as patterns emerging from relationships across a system.
When organizations begin managing themselves as systems, they start asking different questions. Questions more aligned with future-proofing than only managing risk in the moment.
- Do we possess enough foresight and resilience to adapt to external change?
- Do we have the requisite variety within the organization to match the complexity outside it?
- Are we thinking about diversity not merely as a moral aspiration or branding exercise, but as an existential requirement for survival?
- Can we anticipate multiple futures — from the dramatic “black swan” events to the slower-moving transformations already unfolding around us that our mental models may simply not permit us to see?
- Can we zoom in and zoom out? Can we perceive both the trees and the forest, across different spatial scales and timescales?
These are not abstract philosophical questions. Increasingly, they are pressing operational questions.
One of the great paradoxes of complex systems is that stability at the level of the parts often produces fragility at the level of the whole. This sounds counterintuitive, but the pattern appears everywhere. Highly optimized supply chains function beautifully, until disruption arrives. Financial institutions hedging bets at the branch or departmental level can collectively find themselves being left behind. Monoculture agriculture maximizes efficiency and near-term yield while increasing vulnerability to disease and collapse. Bureaucracies that eliminate variation and experimentation become incapable of adapting to changing environments. Assiduous brand alignment can be catalytic in the short term, but catastrophic in the long-term.
In The 55 Minutes, I describe this paradox in organizational terms:
“If the departments of a government or a corporation are in lock step with prescribed rules — they do everything by the book, have very little turnover, and are utterly reliable and predictable in their behavior — then, counterintuitively, this spells trouble for the whole organization. It means that the organization as a whole is ill-prepared to adapt to changes in the external environment.”
The inverse is also true: The instability of the parts – quiet experimentation and improvisation of individuals, the unleashed creativity of teams, even the rogue-ness of entire departments (so long as they are acting in good faith and for the betterment of the organization) – paradoxically produces stability of the whole. This does not mean chaos. It means variation. Experimentation, redundancy, semi-autonomy. Diverse ways of sensing and responding to change. Exploring the edges of possibility, away from the comforting isomorphism of doing things as expected or prescribed. This helps explain, for example, why the university, as an institutional form, has endured for over a millennium.
Nature discovered this principle long ago. Evolution depends upon localized instability – mutation, experimentation, recombination, failure. Without variation among the parts, there is no adaptation of the whole. The same is broadly true of institutions navigating the twenty-first century: Systems optimized exclusively for efficiency, predictability, and control often sacrifice the very adaptability required for survival. A systems-led approach shifts the goal from the chimeric “fail-safe” toward “safe-to-fail” interventions, embracing small-scale, iterative probes to learn how the system responds.
Better yet, it strives for “antifragility.” As Nassim Nicholas Taleb coined, antifragile systems don’t just withstand shocks—they actually thrive and grow stronger because of disorder and volatility. To help cultivate this antifragility, risk managers can employ futures tools like Premortem Analysis. Instead of traditional strategic planning, a premortem asks a team to imagine that their project or organization has completely failed in the future and then work backward to unearth the complex feedback loops and hidden dynamics that caused the collapse. It breaks groupthink and illuminates the unintended consequences, or “fixes that fail,” which so often follow well-intentioned solutions.
Innovation and risk management might seem like two ends of a polarity from up close, but zoom back far enough, and you’ll see the syncopation take shape. It’s not free jazz. It’s just enough structure to scaffold a better tune; New forms, modes, services and products that are future-fit. Indeed, a systems approach to risk asks us to be more careful and thoughtful in how we consider innovation. To move from a heedless rush to action toward asking deeper questions, considering layers ‘above’ and ‘below’ what presents itself at eye level, and to imagine a far greater array of potential consequences.
And perhaps this is the deeper lesson behind the Queen’s question. The greatest risks are the ones we cannot see when we’re overinvested in the narrow here and now. Stability feels comforting. But it is always an illusion.
